New Dental Patient Information – Geneva, OH
Our Team Welcomes You and Your Family
Dr. Owens and Dr. Salen could not be more excited for you to join our dental office and fulfil all of your oral health needs in one location. As you learn more about our dental office, we hope that you take a moment to confirm everything that you need to get started as a new patient. This includes what to expect when you first arrive, forms to fil out, and financing information to keep in mind. We can’t wait to serve you and your family very soon!
Your First Visit
When you first arrive, you’ll be met with a warm greeting from our front desk and given a chance to fill out new patient forms if you have not yet already. If you are attending to have a basic hygiene treatment completed, we’ll capture detailed X-rays, have one of our friendly and experienced hygienists clean your teeth, then one of our dentists will perform a detailed exam. If there is a problem with your tooth, we’ll do everything we can to fix it and stop any present discomfort.
New Patient Forms
The more we learn about you, the better we’ll be able to satisfy all of your oral health needs. When you first arrive, we ask that you fill our necessary new patient forms. Make sure to arrive 15 minutes ahead of your scheduled appointment so you have time to fully complete them. Then, hand it in to our front desk and we’ll call you shortly after. Your information gets passed on to one of our dentists so they can address any of your concerns during the exam.
In House Membership Program
Do you lack dental insurance? You don’t have to miss out on your most essential dental services just because you don’t have coverage. By paying a flat annual rate, you can get guaranteed treatments without having to worry about waiting periods, annual maximums or deductibles. Plus, when you take the discounts into account, you’re paying even less than what can be found at Cleveland Dental Institute. Not only are you saving money, but you’re working with dentists who have more experience overall.
Dental Insurance Welcome
Having dental insurance generally means that you can get the most important forms of care done and out of the way, especially preventive care like exams and cleanings. Dr. Owens and Dr. Salen are happy to accept just about every PPO dental insurance plan out there and are in-network with many major insurance companies. If you have questions about your coverage, our team is happy to look over your plan in more detail.
Just a few of the many insurance providers we work with:
- Lincoln Financial
- Medical Mutual

- United Concordia
- United Healthcare
- and many more!
If you don’t have dental insurance, your plan is not covering all of the cost of your treatment, or you need treatment that is not typically covered by insurance, such as cosmetics, consider CareCredit! This program lets you pay for dental care in monthly installments, rather than all at once, ensuring that you don’t have to break the bank just to get the care that you want. In many cases, there’s even low or zero interest on top of your payment plan!
